About the role
Provide evidence-based primary care remotely for patients 18 years and older. Collaborate with care team to improve patient outcomes in the Western U.S.
What you’ll do
• Perform accurate patient history and physical assessments. • Evaluate and treat medical needs with safe, high-quality care. • Counsel and educate patients and families. • Document patient care in electronic health records. • Coordinate care with internal and external colleagues.
What you’ll bring
• Active Nurse Practitioner license in good standing. • National Board Certification in Family Practice. • 5+ years primary care experience. • Effective communication and organizational skills. • Ability to work remotely and maintain multi-state licensures.
