About the role
Support patients in navigating healthcare pathways, removing barriers, and ensuring timely care. Provide compassionate communication and proactive coordination to improve patient outcomes.
What you’ll do
• Respond to inbound calls and conduct proactive outreach. • Assist with appointment scheduling and transportation coordination. • Identify and address social determinants of health. • Document patient interactions accurately and comply with HIPAA. • Collaborate with internal teams and escalate complex cases.
What you’ll bring
• High School Diploma or G.E.D. required. • 2+ years customer service or healthcare navigation experience. • Strong communication and active listening skills. • Comfortable with multiple systems and accurate documentation. • Commitment to confidentiality and compliance.
