Integrated Client Navigator

About the role
Support continuity of care for facility-managed clients under the BRIDGE-N Project. Improve service quality and access to integrated health services.
What you’ll do
• Link clients to HIV, TB, maternal and child health services. • Prioritize missed appointments and treatment interruptions. • Confirm referral completion and coordinate handovers. • Coordinate with clinical, pharmacy, laboratory and data teams. • Maintain client lists and document contacts and outcomes.
What you’ll bring
• SSCE with 3 years client-navigation or related experience. • Or diploma in Nursing, Community Health or related field with 1 year experience. • Accurate recordkeeping and respectful engagement. • Ability to work collaboratively with facility and community teams. • Commitment to quality integrated health services.
