About the role
Provide in-home health evaluations to patients, building trusted relationships and improving health outcomes. Conduct visits including medication review, physical evaluation, and diagnostic tests as ordered.
What you’ll do
• Conduct 45-minute in-home health evaluations. • Review medication and medical history. • Perform physical evaluations and diagnostic tests. • Use iPad and clinical workflow for evaluations. • Travel locally and conduct virtual visits as needed.
What you’ll bring
• Active, unrestricted license(s) in coverage area(s). • Board certification required. • Multi-state licenses preferred. • Ability to work in diverse home environments. • Full-time availability.
