About the role

What you’ll do
• Design home-based care model and team roles. • Translate evidence into practical care pathways. • Lead clinical design of population health programs. • Partner on quality, workforce, and clinical education. • Co-design clinical workflows with Product and Technology.
What you’ll bring
• MD or DO with active, unrestricted license. • Board certified in Family, Internal, or Geriatric Medicine. • 10+ years senior leadership in interdisciplinary care models. • Expertise in clinical operations and quality improvement. • Experience with Medicare populations and value-based care.
