About the role
Provide comprehensive case management for complex medical cases remotely. Coordinate care to prevent hospital readmissions and ensure integrated care plans.
What you’ll do
• Provide holistic case management interventions. • Coordinate care across multiple providers and programs. • Serve as primary contact for beneficiaries. • Promote appropriate utilization of care resources. • Reduce fragmentation of clinical services.
What you’ll bring
• U.S. citizenship required. • Active RN license in state of employment. • 2 years case management experience. • 3 years clinical or managed care RN experience. • Ability to work M-F, 8 a.m. - 6 p.m. ET.
