About the role
Support patients throughout their care journey, monitor engagement, and coordinate care transitions. Ensure smooth communication between patients, providers, and facilities.
What you’ll do
• Monitor patient lead-up requirements and paperwork. • Follow up on patient requirements and barriers. • Support patient engagement during first 30 days. • Coordinate escalation and transitions to higher care. • Maintain accurate documentation and follow established workflows.
What you’ll bring
• High school diploma or equivalent required. • 1–2 years experience in care coordination or support. • Strong communication and organizational skills. • Ability to manage multiple patients and priorities. • Comfortable working independently and with technology.
