About the role
Provide behavioral health therapy to patients with chronic disease and behavior change needs. Collaborate with a coordinated care team to deliver integrated metabolic care.
What you’ll do
• Provide direct behavioral health care using evidence-based approaches. • Conduct virtual visits and manage caseload. • Document patient encounters and coordinate follow-up care. • Support individualized care pathways for lifestyle change. • Collaborate with dietitians, NPs, and physicians.
What you’ll bring
• Active unrestricted clinical license (LCSW/LPC/LMFT). • Experience with telehealth. • Experience supporting behavior change or chronic disease patients. • Strong clinical judgment and communication skills. • Ability to work in a multidisciplinary telehealth team.
